Intensive Care Manager

Job Summary
Develop care plans for CHNCT members by conducting assessments, identifying barriers, and coordinating outpatient services across the continuum of care. Act as an advocate by communicating with providers and community organizations to ensure quality, cost-effective outcomes while managing third-party liability issues. Monitor and modify care plans as needed, provide education to selected members, and orient new staff as requested. Maintain knowledge of CT Medicaid and HUSKY Health benefits while exercising independent judgment in decision-making. Requires a current CT RN license, two years of direct clinical experience, and strong communication skills.
Required Qualifications
- Nursing major
- Minimum two years direct clinical care experience as an RN
- Behavioral health experience and knowledge of case management
- Excellent verbal and written communication skills
- Good time management and organizational skills
- Ability to manage continually changing priorities
- Strong computer and keyboard skills
- Ability to exercise independent and sound judgment in decision making
- Managed care and case management experience
- CT RN License must be current and unrestricted
- CCM or other URAC approved certification
- Valid and unrestricted motor vehicle operator's license
- Reliable transportation
- Active Registered Nurse License
- State Motor Vehicle License
Desired Qualifications
- Two to four years post-secondary schooling
- Associate's degree
- Bachelor's degree
- Managed care and case management experience preferred
