Job Summary
Intensive Care Coordinator in Lowell, MA. Provide care planning and coordination of services for youth under 21 using the Wraparound model; facilitate Care Planning Team meetings; coordinate multiple services/supports; assist caregivers with advocacy; collaborate with team to ensure consistent treatment messages; navigate school, court, state agencies (DCF, DMH, DDS), and community resources for youth, young adults, and families. Requirements include a Bachelor’s Degree in Social Work, Psychology, or related field and at least 1 year of direct experience; driving in the community with a valid license and reliable vehicle. Training provided in Motivational Interviewing, Courageous Conversations, ARC, Trauma-Informed Practice.
Required Qualifications
- Bachelor’s Degree in Social Work, Psychology, or related field plus at least 1 year of direct experience working with children, young adults, and/or families.
