Job Summary
Intensive Care Coordinator role in Framingham/Waltham providing care planning and coordination of services for eligible youth under 21 using the Wraparound model; facilitate Care Planning Team meetings, coordinate multiple services and supports; help caregivers learn effective advocacy skills; collaborate with the team to ensure continuity in treatment, services, and messages; attend meetings outside Care Plan Meetings as requested; assist in system navigation with schools, courts, state agencies (DCF, DMH, DDS) and community resources for youth, young adults, and families; requires strong communication, risk assessment, adaptability, and relationship-building; driving in the community is required.
Required Qualifications
- Bachelor’s Degree in Social Work, Psychology, or related field
- at least 1 year of direct experience working with children, young adults, and/or families
- valid driver’s license and access to a reliable vehicle
