Job Summary
Intensive Care Coordinator responsible for identifying social determinants of health needs for AllCare Medicaid, Medicare, and Medicare dual eligible members, navigating community-based services, providing benefit education, and coordinating care across the interdisciplinary care team. Duties include assessment, planning, implementing and coordinating services to ensure timely access to medical, behavioral health, and community resources; advocating for members; documenting objectives in care plans; facilitating communication among members, ICT, providers, and DHS/related partners; coordinating DSNP-related activities and required trainings; and performing on-call coverage with potential on-site and home-office work. Must be able to operate in diverse settings, maintain HIPAA-compliant documentation, and leverage community resources to support self-management for members. Bilingual multilingual capability is preferred; strong computer skills (Microsoft Office) and a client-centered, collaborative approach are essential.
Required Qualifications
- High school diploma or GED
- Six months to one year related experience and/or training in related field
- Valid Oregon Driver’s License and vehicle insurance
- Mental Health First Aid Certification or willingness to become certified within 90 days of hire
- Traditional Health Worker certification through OHA preferred (Requires NPI number)
- Excellent organization and time-management skills
- Knowledge of HIPAA regulations
- Ability to read and interpret safety rules and manuals
- Ability to communicate effectively with team members and customers
- Bilingual in English/Spanish preferred
- Ability to travel locally as needed
