Job Summary
Implement Individual Service Plans and document care for each consumer served. Train Home Managers and Direct Care Staff on program implementation and documentation. Conduct weekly home visits and on-site assessments to evaluate consumer health and safety. Finalize staffing schedule authorizations to ensure prescribed ratios meet approved ISP needs. Coordinate resolution of unusual incidents and communicate medical changes to guardians and State Support Agencies. Manage Administrator-On-Call rotations every six weeks while interacting with county officials and family members. Requires a Bachelor's degree, valid Ohio driver's license, and reliable vehicle.
Required Qualifications

- Bachelor's Degree
- Valid Ohio Driver's License (with 4 or less points)
- Auto Insurance w/Liability Coverage
- Safe and Reliable Vehicle
- Ability to pass a criminal background check
