Integrated Nurse Case Manager/RN for Maternal Health/Perinatal in Winchester, VA and surrounding areas. Responsible for in-person face-to-face assessments and remote telephonic assessments for low and high risk pregnant populations (Maternal Health / Perinatal / OB-GYN / Labor and Delivery / Mother-Baby / Postpartum). Develops, monitors, evaluates, and revises the member's care plan to optimize health across the care continuum; conducts telephonic or face-to-face clinical assessments; identifies high-risk members and coordinates care with the health care team; manages chronic illnesses and co-morbidities; facilitates authorizations/referrals within benefits structure; interfaces with Medical Directors and multi-disciplinary teams; presents cases at case conferences; ensures compliance with regulatory and company policies. Requires RN licensure (Compact or VA), Bachelor’s degree preferred, and 3 years nursing experience with case management and managed care experience preferred. Strong communication and analytical skills. Located in Winchester, Shenandoah, Frederick, Rockingham, VA, with hybrid in-person/remote work expectations.
