Job Summary
Address a wide range of patient mental health needs including depression, anxiety, PTSD, substance use, and acute stress through assessments, clinical care, and resource referrals. Coordinate IMH referrals and care plans while engaging with community resources to enhance follow-up and advocacy. Provide remote consultation and direct service coverage as needed to support team absences or meet patient needs within health centers. Work with the IMH team to identify training needs and social determinants of health trends. This role supports a social justice and equity-driven clinical social work model, ensuring access to same-day and scheduled services. Clinicians serve as community liaisons and active members of the Community Health Team, working closely with local communities to provide support and care.
Required Qualifications
- Current applicable state licensure as Licensed Independent Clinical Social Worker (LICSW)
- Eligibility for state licensure
- 3+ years of clinical practice using evidence-based, equity-rooted methods
- Experience providing brief solution-focused assessment & treatment for depression, anxiety, PTSD, substance use, acute stress, relationship challenges, identity-related distress, and behavior change goals
- Experience working with diverse communities affected by inequality and oppression, with a focus on diversity, equity, and inclusion in clinical practice
- Current applicable state licensure as Licensed Mental Health Counselor (LMHC)
- Relevant experience in case management, crisis services, SBIRT models, or reproductive/LGBTQIA+ clinical settings
Desired Qualifications
- Strong preference for experience providing mental health care to individuals with PTSD and complex trauma
