Integrated High Needs Coordinator

Job Summary
Facilitate CFT meetings and develop Individual Service Plans using comprehensive assessment data and team input. Provide clinical support by consulting on psychosocial conditions, substance abuse interventions, and evidence-based practices to maintain focus on outcomes. Ensure appropriate clinical involvement on teams and assist youth and families in securing needed community resources and informal supports. Maintain accurate documentation of contact, treatment progress, and serious incidents while communicating with primary care, mental health providers, and collateral resources. Develop and revise safety/support plans for families and notify supervisors of potential challenges such as non-compliance or crisis.
Required Qualifications
- 22-year age
- High school diploma or GED
- Five years of full time integrated/behavioral health work experience
- Valid and current Arizona driver's license
- Pass motor vehicle screening
- Fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to obtain upon hire
- Drug screening required
Desired Qualifications
- Bachelor's degree in a related field
- Minimum 1 year of full-time integrated/behavioral health experience
