Job Summary
Provides diagnostic assessment, crisis assessment and intervention, referrals to ongoing care, and educational services for behavioral health within a primary care clinic setting; collaborates with an inter-professional team to address psychosocial factors, co-morbid conditions, and lifestyle management; delivers short- to mid-term behavioral health care across modalities (face-to-face and telehealth), leads universal health screening, and participates in quality improvement activities; requires Master's degree and state licensure (LPCC or LCSW in ND) and credentialing with the organization; demonstrates strong communication, flexibility, and team-based care within a Patient-Centered Medical Home/Primary Care Clinic framework.
Required Qualifications
- Master's degree in Counseling, Social Work, Psychology, or related Behavioral Health Sciences
- License must be in good standing in state(s) of practice
- In North Dakota, must be independently licensed as one of the following: Licensed Professional Clinical Counselor (LPCC) or Licensed Clinical Social Worker (LCSW)
