Job Summary
Conduct behavioral health services in primary care clinics by performing diagnostic assessments, crisis interventions, and long-term counseling for patients with co-morbid medical conditions. Deliver empirically validated care via face-to-face visits and telehealth while managing referrals to specialty providers and tracking population health metrics. Lead universal behavioral health screenings, educate staff on emergency protocols, and collaborate with inter-professional teams to address psychosocial factors influencing patient wellness. Maintain documentation of ethical standards and participate in quality improvement activities to ensure safe, patient-centered care across diverse age groups.
Required Qualifications
- Master's degree in Counseling, Social Work, Psychology, or related Behavioral Health Sciences
- Graduation from an accredited school
- License in good standing in state(s) of practice
- Licensed Professional Clinical Counselor (LPCC)
- Licensed Clinical Social Worker (LCSW)

- Active leadership in universal health screening for behavioral and chemical health conditions that negatively impact overall health and wellness
- Ability to work collaboratively with other team members to ensure quality delivery of care
- Ability to provide education and training to staff regarding management of behavioral emergency situations
- Ability to provide education and training to staff regarding current behavioral health information and how this interacts with overall health
- Ability to deliver empirically validated, short-term behavioral health care services (assessment, consultation, diagnosis, and treatment) to ambulatory primary care patients via both traditional face-to-face visits and innovative care delivery models such as telehealth when necessary
- Ability to work closely with other PCMH or PCC team members to address psychosocial and cultural elements of health and disease pertinent to the patient's ability to manage their acute and chronic conditions
- Ability to appreciate other factors that influence health care (developmental, economic, familial, religious, etc.) and wellness
- Ability to assist the team in effectively managing populations of patients
- Ability to track costs and quality associated with innovative integrated care models
- Ability to implement the highest standards of behavioral health care
- Ability to systemically evaluate the quality and effectiveness of behavioral health care
- Ability to actively participate in quality improvement activities
- Ability to document in a responsible, accountable and ethical manner to promote safe, reliable patient centered care
- Ability to participate in a population health management approach to health care by thinking broadly and innovatively and leveraging multiple resources to assist with direct care of patients with both medical and co-morbid behavioral conditions
- Credentialing through the Allied Health Staff of the organization where you will be working
