Job Summary
Conduct behavioral health services in primary care clinics by performing diagnostic assessments, onsite crisis intervention, and delivering empirically validated short-term care via face-to-face visits or telehealth. Provide educational services to patients and staff on behavioral health management, manage referrals to specialty care, and lead universal health screenings for chemical and behavioral conditions. Collaborate with multidisciplinary teams to address psychosocial elements influencing patient wellness and participate in quality improvement activities. Work within a Patient-Centered Medical Home model serving diverse populations in Worthington, MN, with a full-time weekday schedule.
Required Qualifications
- Master's degree in Counseling, Social Work, Psychology, or related Behavioral Health Sciences
- License must be in good standing in state(s) of practice
- Licensed Professional Clinical Counselor (LPCC)
- Licensed Independent Clinical Social Worker (LICSW)
- Licensed Professional Counselor (LPC)
- Licensed Graduate Social Worker (LGSW)
- Credentialing through the Allied Health Staff of the organization
