Job Summary
Conduct behavioral health services in a primary care clinic setting by providing diagnostic assessment, onsite crisis assessment and intervention, and determining need for ongoing care. Deliver empirically validated, short-term behavioral health care services including assessment, consultation, diagnosis, and treatment to ambulatory primary care patients via face-to-face visits and telehealth. Provide education and training to staff regarding management of behavioral emergency situations and current behavioral health information. Serves as the primary team consultant to promote understanding of the relationship between health and psychological/behavioral factors to patients, families, and other team members.
Required Qualifications
- Master's degree in Counseling, Social Work, Psychology, or related Behavioral Health Sciences
- Graduation from an accredited school
- License in good standing in state(s) of practice
- Licensed Professional Counselor - Mental Health (LPC-MH)
- Certified Social Worker - Private or Independent Practice (CSW-PIP)
- Credentialing through the Allied Health Staff of the organization

- Ability to work collaboratively with other team members to ensure quality delivery of care
- Ability to provide education and training to staff regarding management of behavioral emergency situations
- Ability to provide education and training to staff regarding current behavioral health information
- Ability to explain how behavioral health information interacts with overall health
- Ability to deliver empirically validated, short-term behavioral health care services (assessment, consultation, diagnosis, and treatment)
- Ability to deliver care via both traditional face-to-face visits and innovative care delivery models such as telehealth
- Ability to address psychosocial and cultural elements of health and disease pertinent to the patient's ability to manage their acute and chronic conditions
- Ability to appreciate other factors that influence health care (developmental, economic, familial, religious, etc.)
- Ability to assist the team in effectively managing populations of patients
- Ability to track costs and quality associated with innovative integrated care models
- Ability to implement the highest standards of behavioral health care
- Ability to systemically evaluate the quality and effectiveness of behavioral health care
