Job Summary
Develop and monitor member care plans to optimize health across the continuum, conducting clinical assessments for physical and behavioral needs. Coordinate care with interdisciplinary teams, identify high-risk complications, and manage chronic illnesses while ensuring cost-effective benefit utilization. Facilitate authorizations and referrals, present cases at conferences, and address gap in care management. Travel up to one hour to see members 50 to 75% of the week from a Suffolk, Norfolk, or Virginia Beach residence. Requires an RN license, three years of nursing experience, and managed care preference.
Required Qualifications
- Reside in Suffolk, Norfolk, VA Beach, Portsmouth, VA and the surrounding areas
- Available for travel up to 1 hour to see members approximately 50 to 75% a week
- Associates Degree
- Registered Nurse certification
- 3 years of nursing experience
