Job Summary
Integrated Care Program Supervisor leads a multidisciplinary team to deliver high-quality integrated palliative care in Mississauga, Ontario. Responsibilities include coordinating patient- and family-centered care plans, aligning with MAID wishes, managing indirect and direct care coordination, overseeing quality, safety and risk improvements, building stakeholder relationships, and supporting a team of RNs/RPNs, PSWs, and allied health professionals in a unionized environment. Requires RN/RPN licensure, 5+ years of nursing in the community, 2+ years in management, and palliative care program experience; duties emphasize leadership, care coordination, education, and ensuring care aligns with patient goals. Must possess a valid Ontario nursing license, fit on-call rotation, and a reliable vehicle.
Required Qualifications
- Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario
- Minimum of 5 years nursing experience in a community setting and 2+ years in a management role
- Experience with palliative care patients
- Experience with program development, implementation and evaluation
- Certification or enrollment in CNA certification in Palliative Care Nursing or LEAP or CAPCE
- G driver's license and reliable vehicle
- Availability for on-call rotation after hours and weekends
