Job Summary
Lead an integrated palliative care program as a Supervisor in a one-year contract, overseeing a multidisciplinary team to deliver high-quality integrated care in Mississauga and surrounding Ontario communities. Responsibilities include coordinating care plans with patients, caregivers, and health-system partners; leveraging data-driven insights to optimize service delivery; ensuring alignment with patient- and family-centered goals; supporting team members to work to their full scope of practice; overseeing indirect and direct care coordination; managing quality, safety, and risk, and participating in quality-of-care reviews. The role requires RN/RPN licensure in good standing, 5+ years of community nursing experience with 2+ years in management, program development/implementation/evaluation experience, potential advantage for unionized-teams management, care-coordination experience, a valid G driver's license with a reliable vehicle, and availability for after-hours and weekend on-call rotations. The position is in-person in Mississauga, Ontario, Canada, on a contract basis.
Required Qualifications
- Registered Nurse/Registered Practical Nurse (RN/RPN) in good standing with the College of Nurses of Ontario
- Minimum of 5 years nursing experience in a community setting and 2+ years in a management role
- Must have experience working with palliative care
- Experience with program development, implementation and evaluation
- Experience managing teams in a unionized environment an asset
- Experience working with care coordination
- Valid G driver's license and reliable vehicle
- Must be available for on-call rotation after hours and weekends
