Integrated Care Coordinator-SCN

Job Summary

Conduct front office operations including patient registration, scheduling, appointment confirmations, and insurance verification. Perform SCN screenings to assess health-related social needs and connect individuals with community resources such as food assistance, housing, and transportation services. Manage nutritional health initiatives by providing personalized education, counseling, and outreach on healthy eating habits and disease prevention. Document client eligibility, outreach activities, case notes, and referral outcomes using designated technology platforms. Monitor outstanding tasks, referrals, and authorizations while collaborating with providers, navigators, and community organizations to support patients with complex needs. Maintain accurate, compliant records and assist patients in overcoming barriers to healthcare access and treatment compliance.

Required Qualifications

  • College degree
  • Adequate hearing to perform job duties in person and over the telephone
  • Ability to communicate clearly in person and over the telephone
  • Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens
  • Ability to bend, reach, stand, walk, squat, push, and pull exerted regularly throughout a regular work shift
  • Prolonged periods of sitting at a desk and working on a computer
  • Strong organizational and multitasking abilities
  • Excellent communication and interpersonal skills
  • Familiarity with medical terminology and basic understanding of healthcare procedures
  • Proficiency in using office software and electronic health record (EHR) systems
  • Strategic and creative thinking
  • Solution-oriented mindset
  • Comfortable working in group settings
  • Reliable when it comes to attendance and punctuality
  • Ability to learn new things in stated timeframe

Desired Qualifications

  • Experience in healthcare administration, patient navigation, social services, care coordination, or front office operations
  • Knowledge of healthcare workflows, Medicaid populations, community resources, and social care programs
  • Bilingual skills
