Job Summary
Plan, coordinate, and monitor individualized treatment plans to align with behavioral health goals while closing gaps and tracking progress. Deliver comprehensive care management, including health promotion, referrals to community services, and coordination with care teams. Complete Care Management Comprehensive Assessments, ensure clients receive physical exams and medication monitoring, and maintain medical record compliance. Monitor HEDIS gaps, verify enrollment status, and develop person-centered care plans addressing Social Determinants of Health. Assist the Nurse Practitioner with clinic documentation, facilitate on-site operations, manage clinic rosters, and provide health education to clients. Coordinate follow-up services for hospitalizations or life transitions, identify crisis needs, and participate in post-crisis debriefing. Establish collaborative relationships with providers and agencies, attend team meetings, and ensure compliance with state regulatory requirements.
Required Qualifications
- A Bachelor's Degree in any discipline
- Experience working with children and families in case management type/community resource position
- A MH/SU license (including associate-level)
- Certification by the NC Substance Abuse Board
- An RN AND four years of full-time experience working with the MH/SU/IDD population
- A master's degree in a human service field AND at least one year of full-time experience working with the MH/SU/IDD population
- A bachelor's degree in a human service field AND at least two years of full-time experience working with the MH/SU/IDD population
- A bachelor's degree in a non-human service field AND at least four years of full-time experience working with the MH/SU/IDD population
- Two years of experience working directly with individuals with behavioral health conditions
- Two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience
