Job Summary

Conduct behavioral health assessments, diagnoses, and treatment planning while coordinating care within an interdisciplinary team. Collaborate with leadership and clinical staff to establish workflows, improve access to services, and promote whole-person, patient-centered care. Provide psychotherapy across the lifespan, managing crises and documenting progress in electronic health records. Requires current Minnesota licensure and experience with depression, anxiety, trauma, and substance use disorders. This role supports CCM Health's Integrated Behavioral Health program, offering NHSC approval, full health insurance, and on-site child care.

Required Qualifications

  • Master's or Doctoral degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology from an accredited institution
  • Current Minnesota independent licensure (LICSW, LPCC, LMFT, or Licensed Psychologist)
  • Knowledge of diagnostic criteria and evidence-based behavioral health practices
  • Strong clinical assessment, crisis intervention, and treatment planning skills
  • Excellent communication and interdisciplinary collaboration
  • Ability to develop clinical workflows and support program implementation
  • Strong organizational, documentation, and problem-solving skills
  • Proficiency with electronic health records
  • Minimum of two years of post-licensure behavioral health experience
  • Experience providing psychotherapy across lifespan
  • Experience working with individuals experiencing depression, anxiety, trauma, substance use disorders, chronic illness, and severe mental illness

Desired Qualifications

  • Training in Integrated Behavioral Health, Collaborative Care, Primary Care Behavioral Health, or related model
  • Training in evidence-based therapies including CBT, Motivational Interviewing, Solution-Focused Therapy, and Trauma-Informed Care
  • Experience in integrated primary care or collaborative care
  • Experience with program development, quality improvement, or workflow implementation
