Job Summary
Conduct individual assessments and provide evidence-based behavioral health interventions to primary care patients, including skills training and coaching for ongoing management. Diagnose common mental health problems and address issues related to physical or medical conditions through modalities such as CBT, ACT, and psychodynamic psychotherapy. Coordinate with multidisciplinary teams and community partners to facilitate smooth transitions for patients requiring specialized services outside the clinic. Promote health behavior change through brief interventions and education while collaborating on care plans for children, adolescents, adults, and geriatric populations.
Required Qualifications
- Master of Social Work, Master in Professional Counseling, Master of Family Therapy degree from a nationally accredited institution, fulfilling the requirements for licensure in the state of Oregon and/or Washington
- Minimum of two years of clinical experience or healthcare related field
- Must hold a current valid Washington License as LCSW, LICSW, LPC, LMHC or LMFT

- Ability to diagnose common mental health problems
- Able to conduct solution focused interventions
- Modalities including ACT, CBT, CBT-I, and psychodynamic psychotherapy
- Recognizes and advocates for patients regarding behavioral health strategies/interventions
- Promotes health behavior change through evaluation, brief intervention and education
- Works with multidisciplinary team to help support the patient's health
- Coordinates with LMG providers and community partners to refer for appropriate mental health therapy
- Demonstrates competencies in assessment of emotional, cognitive and behavioral disorders in child, adolescent, adult and/or geriatric patients
- Demonstrates competency in assessment of cognitive and psychological problems associated with, and/or exacerbating medical conditions
- Experience with working with an interdisciplinary team
- Skilled in case conceptualization and consultation
- Flexibility and willingness to work at the fast pace of primary care
