Job Summary
Conduct individual assessments and provide recommendations to the primary care team to enhance patient care plans. Deliver ongoing skills training and coaching for patients requiring multiple sessions to improve health and functioning. Support transitions to community partners for patients needing services beyond the clinic. Diagnose common mental health problems and apply modalities including ACT, CBT, CBT-I, and psychodynamic psychotherapy. Coordinate with multidisciplinary providers to refer for appropriate therapy and advocate for behavioral health strategies. Assess cognitive and psychological problems associated with medical conditions across child, adolescent, adult, and geriatric populations.
Required Qualifications
- Master of Social Work, Master in Professional Counseling, Master of Family Therapy degree from a nationally accredited institution, fulfilling the requirements for licensure in the state of Oregon and/or Washington
- Minimum of two years of clinical experience or healthcare related field
- Must hold a current valid Oregon and/or Washington License as LCSW, LICSW, LPC, LMHC or LMFT
- Ability to diagnose common mental health problems
- Able to conduct solution focused interventions
- Modalities including ACT, CBT, CBT-I, and psychodynamic psychotherapy
- Recognizes and advocates for patients regarding behavioral health strategies/interventions
- Promotes health behavior change through evaluation, brief intervention and education
- Works with multidisciplinary team to help support the patient's health
- Coordinates with LMG providers and community partners to refer for appropriate mental health therapy
- Demonstrates competencies in assessment of emotional, cognitive and behavioral disorders in child, adolescent, adult and/or geriatric patients
- Demonstrates competency in assessment of cognitive and psychological problems associated with, and/or exacerbating medical conditions
- Experience with working with an interdisciplinary team
- Skilled in case conceptualization and consultation
- Flexibility and willingness to work at the fast pace of primary care
