Job Summary
Conduct individual patient assessments and provide evidence-based behavioral health interventions within the primary care team. Deliver ongoing skills training and coaching to improve patient functioning when indicated. Coordinate with community partners to facilitate smooth transitions for patients requiring specialized services outside the clinic. Partner with multidisciplinary providers to enhance care plans and promote health behavior change through brief interventions and education. Requires LCSW, LPC, or LMFT licensure in Oregon or Washington, with two years of clinical experience. Supports patient-centered care for emotional and behavioral disorders across child, adolescent, adult, and geriatric populations.
Required Qualifications
- Master of Social Work, Master in Professional Counseling, Master of Family Therapy degree from a nationally accredited institution, fulfilling the requirements for licensure in the state of Oregon and/or Washington
- Minimum of two years of clinical experience or healthcare related field
- Ability to diagnose common mental health problems
- Able to conduct solution focused interventions
- Modalities including ACT, CBT, CBT-I, and psychodynamic psychotherapy are all applicable
- Recognizes and advocates for patients regarding behavioral health strategies/interventions
- Promotes health behavior change through evaluation, brief intervention and education
- Works with multidisciplinary team to help support the patient's health
- Coordinates with LMG providers and community partners to refer for appropriate mental health therapy
- Demonstrates competencies in assessment of emotional, cognitive and behavioral disorders in child, adolescent, adult and/or geriatric patients
- Demonstrates competency in assessment of cognitive and psychological problems associated with, and/or exacerbating medical conditions
- Experience with working with an interdisciplinary team
- Skilled in case conceptualization and consultation
- Flexibility and willingness to work at the fast pace of primary care
- Licensed in one of the following within applicable state: Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW), Licensed Professional Counselor (LPC), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT)
