Job Summary
Coordinate incoming calls and referrals from health professionals, agencies, and families to initiate care services. Complete intake screenings by documenting required information, analyzing data for admission decisions, and entering patient files into the management system. Act as a liaison with external sources, field staff, and physicians to arrange appointments and receive verbal orders where permitted. Promote company services to prospective clients, manage community outreach activities, and support employee hiring and file maintenance. Adhere to nursing standards and applicable laws while maintaining confidentiality of patient health information.
Required Qualifications
- Bachelor's degree in business, Marketing, Healthcare or related discipline, or an equivalent combination of education and experience
- Three (3) years relevant experience in medical services or clinical environment
- knowledge of medical terminology
- previous intake experience of at least one year

- Ability to work flexible schedule and/or evening hours as needed
- Ability to sit in front of CPU for long periods of time
- occasional lifting or carrying up to 10 lbs
Desired Qualifications
- Experience working with Insurances, family caregiver programs, community resources, waiver programs, Dementia programs is a plus
