Job Summary
Conduct intake assessments for new clients, ensuring all required paperwork is completed and progress notes are entered into electronic systems accurately and on time. Familiarize clients with the site and program requirements while providing information and referral services as specified in their service plans. Advocate for clients by assisting in navigation of the service delivery system to ensure receipt of entitlement services. Availability to provide evening and weekend coverage as needed.
Required Qualifications
- High School Diploma or equivalent OR equivalent experience and skills
- Effective oral and written communication skills
- Excellent interviewing skills
- Excellent time management and organization skills
- Availability to provide evening and weekend coverage as needed
- Computer literacy, particularly with Microsoft applications
Desired Qualifications
- Prior experience in case management, assessment counseling, and crisis intervention
