Job Summary
Screen and assess applicants for emergency shelter, SUS/Palladia Veterans Services, and Homebase, serving as the primary initial contact for Veterans and their families. Schedule and conduct intake and follow-up appointments via telephone, in person, and in the field at DHS installations and outreach locations. Navigate applicants through the intake process, document screenings in DHS CARES and AWARDS systems, and collect required enrollment documentation. Assess immediate service needs, provide referrals for both in-house and external supports, and assemble intake reports. Notify households of intake decisions and respond to information requests from internal SUS staff and DHS.
Required Qualifications
- Bachelors' degree or higher preferred
- Out in the field approximately 75% of the time throughout Nassau and Suffolk County
- Required to meet Veterans out in the field, either using their personal vehicle or the company vehicle
- A clean driving record is required is using a company vehicle
- Willingness to attend occasional events outside of normal business hours
- Ability to work on Veterans Day
Desired Qualifications
- Former service in the U.S. Armed Forces
- past experience with working with U.S. military veterans
