Intake Specialist (INTSPC-02)

Job Summary
Conduct comprehensive intake assessments, document diagnoses, and complete individualized treatment plans aligned with member goals. Facilitate all intake paperwork, consents, and admission documentation while educating members on program expectations, policies, rights, and responsibilities. Ensure compliance with confidentiality and regulatory requirements, provide orientation to new admits, and collaborate with clinical staff to ensure seamless transitions into care. Maintain accurate records within electronic health record systems and meet established KPIs focused on member care, engagement, and intake timeliness. Participate in ongoing clinical supervision and required continuing education.
Required Qualifications
- Master's degree in Social Work or related field
- Minimum of 4 years of experience in behavioral health, including intake assessments and treatment planning
- Strong communication, organizational, and critical thinking skills
- Experience with motivational interviewing and client engagement
- Ability to work both independently and as part of a team
- Must be 21 years of age or older
- Ability to pass background check, drug screening, and TB testing
