Intake Spec (25511)

Receive initial phone inquiries and written referrals, then gather pertinent information to determine eligibility for services. Meet with clients and families to communicate service details, obtain signatures on releases of information, and complete intake paperwork. Enter referral data, demographic details, and status changes into the database while preparing clinical records and admission documents. Refer to alternate service providers as needed and schedule admissions in collaboration with families and the program. Maintain general knowledge of all services to address caller needs, including emergency situations. This role reports to the Executive Program Director and requires onsite work.
