Job Summary
Perform specified distributive and clinical pharmacy activities and related services as they pertain to providing safe and effective home infusion therapies. Responsibilities include supervising pharmacy staff, completing intake pharmacy activities, working with multiple departments to coordinate operations, conducting initial patient assessments, and maintaining documentation. Preferred qualifications include a Bachelor's or Doctorate in Pharmacy and a minimum of two years' experience in a home infusion pharmacy.
Required Qualifications
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD preferred) from an accredited college of pharmacy
- Currently licensed as a registered pharmacist in state of employment
- A minimum of two years’ recent experience in a home infusion pharmacy preferred
- Experience and strong knowledge base in intravenous admixture services and intravenous therapy
- Ability to evaluate and monitor drug therapy in collaboration with healthcare professionals
