Job Summary
Conduct community outreach and perform initial eligibility assessments using the Uniform Assessment System (UAS) tool for potential new members. Evaluate safety and care needs to formulate initial plans of care, then communicate with clinical teams, referral sources, and physicians to ensure seamless transitions to managed long-term care. Complete enrollment documentation timely while serving as a community resource for intake issues. This mobile role requires traveling over 50% of the time to homes and offices within a specific territory, with flexible hours including evenings and weekends. Candidates must possess a current New York RN license, valid driver's license, and bilingual skills in Spanish, Russian, or Chinese.
Required Qualifications
- Associates degree in Nursing
- One (1) year experience working in an acute care setting
- Current Registered Nurse license in the state of New York
- Valid driver's license, vehicle and verifiable insurance
- Ability to speak English
- Ability to read, write, and speak English
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals
- Knowledge of Medicare and Medicaid regulations
- Must reside in the same territory they are assigned to work in
- Must be able to lift 50 lbs
