Job Summary
Conduct member intake and assessments to determine placement requirements and identify necessary housing, health services, and non-housing supports. Collaborate with Enhanced Care Managers and Operations Manager to follow up on resource referrals, manage case assignments, and monitor member progress. Update member files, prepare documentation reports on intake numbers, and utilize provider portals to verify status information. Mediate between partnered health plans and members while participating in weekly meetings to update teams on policy changes and member statuses. Build professional relationships with multiple providers and make intake calls as assigned to reach team goals.
Required Qualifications
- High school, diploma or GED
- Able to lift up to 15lbs
- Excellent communication, interpersonal, customer service and organizational skills
- Proficiency in multitasking, organization, and attention to detail
- Ability to work in a team environment and independently
- Proficiency in Microsoft Office Suite, data entry and electronic systems
- Computer skills for documentation, email and communications
- Successful pre-employment drug screening
- Successful background check
Desired Qualifications
- Previous experience in case management, social work, or healthcare coordination
- Previous experience handling multiple clients over the phone
- Bilingual in Spanish
