Intake Coordinator - San Bernardino

Conduct screening interviews at the center or community agencies to determine eligibility for the rehabilitation program, including phone screenings and conferring with referral agents. Process admitted beneficiaries by orienting them to services, obtaining signatures on agreements, entering data into the ARC software, and managing case records and dormitory assignments. Maintain ongoing contact with referral sources, dedicating at least four hours weekly to community outreach using an ARC vehicle, and refer ineligible applicants to appropriate community agencies. Prepare required reports and statistical information while maintaining strict confidentiality of all beneficiary records in compliance with CFR 42 and HIPAA. Attend inter-agency meetings as needed and crosstrain to cover Program Aide tasks when assigned.
