Job Summary
Intake Coordinator to be the first point of contact for prospective clients and referral sources, guiding new clients through the intake process, assessing needs, and connecting them with services that best support their goals. Responsibilities include managing incoming referrals and client inquiries, conducting intake assessments and gathering required documentation, educating clients and families on available services and eligibility requirements, coordinating with internal teams to ensure appropriate service placement, verifying information and maintaining accurate records, tracking intake progress, and providing exceptional customer service and follow-up throughout onboarding.
Required Qualifications
- Strong communication and relationship-building skills
- Excellent organizational and multitasking abilities
- Experience in healthcare, social services, customer service, or intake coordination preferred
- Ability to work efficiently in a fast-paced, client-focused environment
