Intake Coordinator - LPN or LVN

Job Summary
Coordinate patient referrals and serve as a resource for referral sources while collaborating with branch and sales teams to ensure adequate coordination and transition to services. Apply and demonstrate knowledge of regulatory guidelines and governing processes, including Medicare guidelines that govern home health or hospice. Maintain a valid state driver license with automobile liability insurance and dependable transportation in good working condition to safely drive in all weather conditions. Requires a high school diploma, two years of healthcare industry experience, and one year in home health or pediatrics. Must be a licensed LPN or LVN with basic technology skills.
Required Qualifications
- High school diploma or equivalent
- At least two years of experience in the health care industry
- One year experience in home health or pediatrics
- Demonstrated experience in customer service
- Knowledge of Medicare guidelines that govern a home health or hospice
- Basic demonstrated technology skills, including operation of a mobile device
- Graduate of an approved school of professional nursing
- Licensed in the state in which they currently practice as an LVN or LPN
- Valid state driver license
- Automobile liability insurance as required by law
- Dependable transportation in good working condition
- Ability to safely drive an automobile in all types of weather conditions
