Job Summary
Intake Coordinator responsible for handling incoming home health referrals, verifying insurance eligibility, obtaining authorizations, and coordinating with physicians, discharge planners, and clinical teams to ensure a seamless transition to home health care. Will use Homecare Homebase (HCHB) to input patient data, manage workflows, and track referral status, while ensuring compliance with federal, state, and insurance criteria. Requires 2+ years in Medicare-certified home health, 1+ year in intake/referral processing or insurance verification, and proficiency with HCHB. Focused on maintaining clear communication with referral sources, families, and internal staff; coordinating with scheduling for timely nursing/therapy assessments.
Required Qualifications
- Home Health Experience: Minimum of 2 years of direct experience working within a Medicare-certified home health agency
- Intake Experience: Minimum of 1 year of dedicated experience in home health intake, referral processing, or insurance verification
- Software Proficiency: Minimum of 1 year of hands-on experience using Homecare Homebase (HCHB)
