Job Summary
As an Intake Coordinator, you will manage end-to-end referral intake for in-home health services, validating completeness, verifying insurance eligibility, securing initial authorizations, and ensuring compliance with Medicare, state, and payer requirements. You will enter and track referral activity in Homecare Homebase (HCHB), collaborate with scheduling to support timely start-of-care visits, and communicate clearly with referral sources, patients’ families, and internal teams. The role emphasizes precise intake processing, workflow coordination, and maintaining audit-ready documentation to prevent delays in care. Required experience includes Medicare-certified home health exposure, referrals/intake/verification experience, and proficiency with HCHB.
