Job Summary
Direct and manage all daily patient referral, intake, and scheduling operations, including oversight of intake policies, procedures, and workflow efficiency. Verify insurance coverage, obtain authorizations, and support maximum third-party reimbursement by negotiating service pricing within established guidelines. Coordinate seamless patient transitions by overseeing intake, education, plan of care initiation, and collaboration with multiple service providers. Serve as the primary point of contact for inbound calls, responding promptly to patient, staff, and referral source inquiries. Facilitate daily intake or staffing stand-up meetings and maintain accurate data on staff demographics, skill sets, and availability to align patient needs with resources. Ensure compliance with all state, federal, Medicare, and Joint Commission regulatory requirements related to intake, scheduling, and service delivery. Monitor, gather, and report referral and staffing metrics, including trends, productivity, and performance indicators.
Required Qualifications
- Experience as a scheduling/intake coordinator
- Compassionate demeanor
- Knowledge of state, federal, Medicare, and Joint Commission regulatory requirements related to intake, scheduling, and service delivery
- Ability to verify insurance coverage and obtain authorizations
- Ability to negotiate service pricing within established guidelines
- Proficiency with Home Care Home Base
- Ability to manage clinician scheduling and staffing processes
- Ability to maintain accurate data on staff demographics, skill sets, availability, and productivity levels
- Ability to respond promptly and professionally to patient, staff, and referral source inquiries and concerns
- Ability to establish and maintain strong relationships with referral sources, patients, families, clinical staff, contract staff, and internal teams
- Ability to facilitate daily intake or staffing stand-up meetings
- Ability to accurately complete and manage all scheduling-related workflows, including new referrals, missed visits, visit adjustments, recertifications, caseload reassignments, and staffing changes
- Ability to provide staffing solutions for new patient referrals
- Ability to identify needs for additional staff resources
- Comprehensive knowledge of contractual relationships with payers and contract staff
- Ability to monitor, gather, and report referral and staffing metrics, including trends, productivity, and performance indicators
- Ability to assist leadership with budget planning, resource allocation, and continuous process improvement initiatives
- Demonstration of professionalism, accountability, and commitment to ongoing growth and competency development
