Intake Coordinator

Direct all daily patient referral and intake operations, including establishing and implementing intake policies. Initiate charts into the EMR and maintain workflow while tracking census, pending admissions, and care plan meetings on the stand-up board. Ensure compliance with state, federal, and CHAP regulatory requirements for referrals and intakes. Establish relationships with referral sources to enhance perceptions of Heart to Heart Hospice as a high-quality provider. Gather and report referral statistics, including key customer referral trends, and participate in strategic planning to identify improved services. Maintain knowledge of contractual relationships to ensure admissions adhere to contract provisions and assist sources in accessing community resources when needed. Submit all documentation timely and perform other duties as required.
