Job Summary

Intake Care Coordinator responsibilities include conducting initial patient assessments and gathering necessary medical and insurance information; verify insurance coverage and obtain prior authorizations; schedule and coordinate home infusion services; maintain accurate patient records; communicate with other departments regarding referral status; notify patients/families regarding coverage and payment responsibilities; collaborate with the clinical team to ensure smooth transitions and continuity of care.

Required Qualifications

  • High school diploma or equivalent; associate or bachelor’s degree preferred
  • Previous healthcare-setting experience, preferably in home infusion or pharmacy services
  • Knowledge of HCPC, ICD10 & CPT codes
  • Proficiency in electronic health records (EHR) and Microsoft Office Suite
  • Excellent communication and interpersonal skills
  • Strong organizational and multitasking abilities
  • Ability to verify benefits and obtain prior authorizations
  • Strong customer service skills
  • Ability to schedule and coordinate home infusion services
  • Compliance with regulatory standards
  • Team collaboration with clinical staff
