Job Summary
Intake Care Coordinator responsibilities include conducting initial patient assessments and gathering necessary medical and insurance information; verify insurance coverage and obtain prior authorizations; schedule and coordinate home infusion services; maintain accurate patient records; communicate with other departments regarding referral status; notify patients/families regarding coverage and payment responsibilities; collaborate with the clinical team to ensure smooth transitions and continuity of care.
Required Qualifications
- High school diploma or equivalent; associate or bachelor’s degree preferred
- Previous healthcare-setting experience, preferably in home infusion or pharmacy services
- Knowledge of HCPC, ICD10 & CPT codes
- Proficiency in electronic health records (EHR) and Microsoft Office Suite
- Excellent communication and interpersonal skills
- Strong organizational and multitasking abilities
- Ability to verify benefits and obtain prior authorizations
- Strong customer service skills
- Ability to schedule and coordinate home infusion services
- Compliance with regulatory standards
- Team collaboration with clinical staff
