Job Summary
Coordinate patient referrals and serve as a resource for referral sources while collaborating with branch and sales teams to ensure adequate coordination and transition to services. Apply and demonstrate knowledge of regulatory guidelines and governing processes. Maintain a valid state driver's license, automobile liability insurance, and dependable transportation in good working condition. Must be able to safely drive an automobile in all types of weather conditions. Requires a high school diploma, two years of health care industry experience, and one year in home health or pediatrics. Medicare guidelines knowledge and customer service experience are essential. Preferred qualifications include an approved nursing school graduate, clinical license, and experience with intake responsibilities at a multi-location company.
Required Qualifications
- High school diploma or equivalent
- At least two years of experience in the health care industry
- One year experience in home health or pediatrics
- Demonstrated experience in customer service
- Knowledge of Medicare guidelines that govern a home health or hospice
- Basic demonstrated technology skills, including operation of a mobile device
- Valid state driver license
- Automobile liability insurance as required by law
- Dependable transportation in good working condition
- Ability to safely drive an automobile in all types of weather conditions
Desired Qualifications
- Graduate of an approved school of professional nursing
- Licensed in the state in which they currently practice
- Registered nurse or licensed professional/vocational nurse
- Experience with intake responsibilities
- Experience working with a multi-location company
