Intake Coordinator - Clinical (Home Care), Full-Time

Process home care referrals by documenting new intake data, verifying patient insurance, and distributing essential background information to appropriate clinicians. Coordinate care with interdisciplinary teams, physician offices, and community liaisons to ensure patients are scheduled for visits and stakeholders are informed of status. Critically assess patient conditions, progress, and ongoing needs while providing clinical guidance and educational opportunities. Respond to incoming calls, answer patient inquiries, and maintain confidentiality in adherence to HIPAA guidelines. Manage multiple tasks independently to meet time deadlines while working closely with referral sources to establish communication.
