Job Summary
Process home care referrals by receiving, documenting, and distributing intake data to appropriate clinicians while coordinating with referral sources, community liaisons, and interdisciplinary teams. Critically assess patient conditions and progress to enhance case manager knowledge, verify insurance benefits, and provide clinical guidance. Maintain confidentiality through strict adherence to HIPAA guidelines while managing multiple tasks, answering incoming calls, and distributing essential background information and plans of care. Work independently to meet time deadlines, participate in clinical activities including faxing and scanning, and respond to patient and agency needs professionally.
Required Qualifications
- Associate's/Technical Degree in nursing or equivalent combination of education/related experience
- Current Registered Nurse or Licensed Vocational Nurse or Licensed Practical Nurse in the state of practice
- All required vaccinations as a condition of employment and annually thereafter, where applicable
- Acknowledgement of E-Verify Participation and Right to Work notices
Desired Qualifications
- Three years' experience in Home Care
