Job Summary
Intake Coordinator role overseeing the initial patient referral process, verifying insurance benefits and authorizations, accurately entering patient data into EMR, and coordinating care with Nursing Supervisors and interdisciplinary team. Responsibilities include receiving referrals, communicating with patients and referral sources, and supporting agency quality initiatives.
Required Qualifications
- Minimum one year of experience in referral intake is preferred
- Experience with third party payers, Medicare and Medicaid highly desirable
- Medical Field background preferred, including familiarity with medical terminology
- Excellent computer and customer service skills
- LVN/LPN preferred
- Valid Driver’s License
- Valid Auto Insurance
- HomeCare HomeBase experience preferred
