Job Summary
Receive and process referrals to the Early Supports for Infants and Toddlers Program, educating prospective clients and referral sources about eligibility criteria. Engage in sensitive conversations with families experiencing complex circumstances to guide them to appropriate services and resources. Build and maintain relationships with hospital, therapist, and school referral sources. Initiate client records and ensure all data-entry within the electronic health record system is complete and accurate. Participate in community events to inform the public of ChildStrive programs and reach prospective clients. Work within the Intake Team to support families birth through three years old with developmental delays or identified disabilities.
Required Qualifications
- 3+ years of experience working in a field of social services or early childhood
- A.A./B.S. degree in a field related
- Oral and written fluency in English and another language
- Knowledge of Microsoft Suite

- Experience with Electronic Health Record
