Job Summary
Coordinate all referrals by receiving inquiries, gathering client clinical and demographic information, and entering data into company software. Evaluate referrals for service appropriateness, notify insurance providers for prior authorization, and assist prospective clients in identifying home care services. Maintain professional relations with referral sources, payers, and clinical staff while ensuring confidentiality and communicating status updates to the Area Manager. Assign referrals to clinicians for assessment and manage patient complaints with independent judgment. Operate office equipment and perform related duties as assigned.
Required Qualifications
- Minimum of three years health care experience
- intake and home health experience
- Knowledge of Medicare, private insurance and Minnesota Medical Assistance regulations
- Ability to promote and maintain a positive attitude in and encourage others to do the same
- Excellent written and oral communication skills
- Strong organizational skills and the ability to work independently with minimal supervision
- Proficient with computers and the ability to learn new software
- U.S. Citizenship or evidence of valid Alien Work Permit
- Pass initial and ongoing background studies and screenings including but not limited to those of the Minnesota Department of Health and the Federal Office of the Inspector General's List of Excluded Individuals and Entities
