Job Summary
Process referrals, assign them to clinicians, and track clients in referral software while performing accurate, detailed data entry. Build relationships with clients and stakeholders, conduct quality control checks, and ensure all protocols, procedures, and workflows are followed. Troubleshoot and resolve issues with clinicians, communicate with team members to ensure information is disseminated, and answer phone calls to direct callers appropriately. Complete ad hoc tasks and maintain up-to-date records to meet targets. Work Monday through Friday, including bi-monthly evening shifts and weekend on-call remote availability. This role supports 1to1 Rehab's mission of providing individualized care across speech-language, physiotherapy, occupational therapy, and nutritional services in the Greater Toronto Area.

- Exhibit superior customer service skills that reflect sensitivity to the needs of clients
Desired Qualifications
- Health Care and Home Care experience is considered an asset
