Job Summary
LPN Intake Coordinator role focusing on office operations and intake workflow in a home health setting. Key duties include answering phones and managing faxes, entering patient referrals into the EMR, coordinating home health services, creating and maintaining forms/records, filing Medicare paperwork and tracking physician orders, assembling admission packets, supporting quality assurance and regulatory compliance, and providing team support as needed. Requires active LPN license, 1+ year home health experience, valid driver’s license, and proficiency with Microsoft Office; ability to participate in on-call rotation. Located in Maitland, FL (and also applicable to Orlando, FL area).
Required Qualifications
- Active LPN license (required)
- Valid driver’s license & reliable transportation
- 1+ year of home health experience (required)
- Experience scheduling home health referrals (preferred)
- Proficiency with Microsoft Office Suite (Word, Excel, etc.)
- Ability to participate in on-call rotation
