Intake Assessment Representative- Weekends

Intake Assessment Representative handles initiating and coordinating referrals for home health services, verifies patient and referral information, confirms initial eligibility for home health services, inputs data into EMR, coordinates scheduling of start of care, and liaises with referral sources, MD offices, insurance teams, and HS departments. Requires coursework in medical terminology or equivalent experience; a Bachelor's Degree is preferred. Candidates should demonstrate strong data-entry, critical-thinking, and written/spoken communication skills, plus proficiency with Microsoft Office and the ability to manage multiple tasks in a high-volume healthcare environment. Knowledge of CMS regulations and home health processes is beneficial.
