Job Summary

Coordinate and process incoming referrals by assessing needs, verifying patient information, and determining initial eligibility for Home Health services. Gather necessary referral and payor data, input it into the electronic medical record, and schedule admission dates while ensuring accuracy and completeness. Act as a liaison between referral sources, medical offices, patients, and insurance teams to resolve eligibility issues and manage fax communications. Triage incomplete referrals, assign appropriate start-of-care dates, and prepare specific reports as directed. Stay current on CMS regulations and Home Services products to maintain service standards and enhance relationships with external partners.

Required Qualifications

  • 2 years Experience in customer service including demonstrated computer proficiency
  • 1 year Experience in a clinical setting or home services
  • 6 months General office experience, preferably in a health care environment
  • 6 months Experience with electronic medical records
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
  • Ability to compute rate, ratio, and percentage
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
  • Ability to write reports, business correspondence, and procedure manuals
  • Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
  • Strong written and verbal communications
  • Ability to communicate in a clear, concise manner both verbally and in writing
  • Excellent computer skills
  • Excellent Microsoft Office Suite software skills
  • Ability to understand, read, write, and speak English
  • Ability to work independently in a high volume environment
  • Ability to think critically and to work with complex detail
