Job Summary
Intake and Admissions Representative responsible for delivering excellent patient-family and referral-source service in hospice, handling referrals via phone/portal/email/fax, creating referral records in the EHR, initiating intake through a virtual platform, scheduling admissions with a 4-hour response goal, retrieving medical records, monitoring referrals, verifying patient benefits, and coordinating with leadership on high-risk referrals. Requires in-office, full-time schedule (8am-5pm, may shift to 7am-4pm), strong communication, multitasking in a fast-paced environment, independent work, and proficiency with phones and basic computer skills.
Required Qualifications
- High school diploma or GED required
- A minimum of 1 year of experience in secretarial or related experience required
- Excellent communication skills (verbal and written)
- Ability to multitask and prioritize in a fast-paced environment with a high level of accuracy
- Ability to work independently; self-motivated
- Proficient at basic phone and computer skills
- Must be comfortable answering phones with a heavy call volume
