Job Summary
Provide ongoing Care Coordination and Insurance Navigator services to link the public and service agencies to clinical services at ElevaCare. Coordinate behavioral and physical health-care needs, assist clients with finding and accessing healthcare services, resources, and insurance options, and monitor the treatment process including intake, referrals, and treatment plan reviews. Coordinate care with internal and external providers, manage a caseload of clients, and ensure confidentiality and data privacy while utilizing tools such as PHQ-9, GAD-7, CSSRS, and other screening tools.
Required Qualifications
- Two-year technical college diploma or equivalent education and experience preferred.
- Experience or ability to use office equipment and technology.
- Strong verbal, written, organizational, and interpersonal skills.
