Job Summary
Coordinate clinical, psycho-social, and spiritual services through a case management model, collaborating with the Interdisciplinary Team to assess patients, educate families, and ensure continuity of hospice care. Explain Medicare benefits, obtain informed consent, and maintain comprehensive care plans while documenting bedside observations and interventions. Provide physical and emotional support during crises, adjust assignments for supervisory duties, and prevent unwanted hospitalizations via anticipatory care. Complete routine and emergency assessments, including participation in on-call services for after-hours support. Requires an active RN license, CPR certification, and experience in hospice or home health.
Required Qualifications
- Associate or Bachelor's degree in Nursing
- Active and unencumbered Registered Nurse license in state(s) of employment
- Current CPR certification
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
- Strong written and verbal communications
- Ability to understand, read, write, and speak English
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
- Ability to compute rate, ratio, and percentage
- Regular standing, walking, and manual dexterity
- Ability to lift and move objects up to 50 pounds
- Visual acuity including close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
- Two (2) years of full time experience as a Registered Nurse (Louisiana specific)
- Two (2) years of full-time clinical experience as a Licensed Practice Nurse in hospice (Louisiana specific)
- One (1) year of full-time experience as a Registered Nurse (Mississippi specific)
- Three (3) years of full-time continuous employment as a Licensed Practical Nurse immediately preceding Registered Nurse licensure (Mississippi specific)
- Ability to articulate and embrace integrated healthcare at home philosophy
